Comprehensive Geriatric Medicine

The Division of Geriatrics at Asan Medical Center takes a holistic approach to identifying and treating the underlying causes of various issues in older adults that are distinct from normal aging, including frailty, weight loss, loss of appetite, dizziness, sarcopenia, and sleep disturbances. In particular, for hospitalized frail older adults, the Acute Care for Elders (ACE) Team, composed of multidisciplinary specialists, provides systematic and expert integrated care.

Core Clinical Services

1. Comprehensive Geriatric Assessment

We conduct multidimensional evaluations for common issues in older adults, such as loss of appetite, weight loss, dizziness, and cognitive decline. This allows us to accurately assess each individual’s health status and develop personalized treatment and care plans optimized for their specific characteristics and circumstances.

2. Medication Optimization and Deprescribing

We carefully review medications taken by older adults to reduce unnecessary prescriptions and minimize the risk of side effects such as falls, delirium, and urinary problems.

3. Preoperative Risk Assessment and Functional Recovery Management

We evaluate the health status of older adults scheduled for surgery to minimize preoperative risks and provide systematic management plans aimed at postoperative functional recovery and quality of life improvement. Each year, more than 1,000 patients in their 80s and 90s undergo high-risk surgeries and anesthesia at Asan Medical Center.

4. Integrated Inpatient Care by the ACE Team

The Division of Geriatrics at Asan Medical Center operates the ACE (Acute Care for Elders) Team for hospitalized frail older adults. This program is based on the 4M principles of an Age-Friendly Health System, which are What Matters, Mobility, Medication, and Mentation, proposed by the Institute for Healthcare Improvement (IHI).

The ACE team develops and implements individualized care plans through multidisciplinary meetings involving physicians, nurses, pharmacists, social workers, and physical therapists to ensure care is optimized for each patient’s condition.
